Pay: $60,000 annually

Expected hours: 40 per week.

Job Overview:

Responsible for the programmatic oversight of the GROW-Associates Day Habilitation Services. Develop and maintain relationships with outside agencies, managing the day-to-day activities of the program, reporting to the Mass Health agency and other involved agencies, monitoring compliance with all applicable laws and regulations governing the program, and implementing the program’s policies and procedures. In absence of Day Habilitation Director assume all duties including supervision of Healthcare Supervisor.

Essential Functions of the Position:

  1. Work with the Director of Day Habilitation in the planning, development and implementation of agency goals and objectives.
  2. Ensure compliance with CARF standards, Mass Health, DDS, agency policies and procedures and any additional statutes and regulations as it pertains to Day Habilitation.
  3. Perform duties to reflect agency/program policies and mission statement.
  4. Assists individuals served in the development and articulation of their needs through the DHSP process, program, family and other providers.
  5. Assist the Director of Day Habilitation with interview and hiring of prospective employees.
  6. Provides supervision of the daily activities of direct care staff; training of direct care staff to ensure implementation of the Day Hab Service Plans and completion of progress notes; including all documentation and maintenance of records to demonstrate compliance with funding source requirement.
  7. Responsible for conducting performance evaluations of all supervised staff.
  8. Maintain an excellent working relationship with state funding agents, community resources, other human service agencies and consumer families.
  9. Assist with monitoring the program’s petty cash and budget. 
  10. Coordinate and conduct staff meetings. 
  11. Responsible for the development of daily staff schedules and assignments.
  12. Participate in Interdisciplinary Team meetings as needed. 
  13. Complete all required documentation in a timely manner.
  14. Assist the Director of Day Habilitation in managing referrals, tours and oversee orientation of new persons served within Day Habilitation.
  15.  Ensure incident reports on HCSIS or other agencies are completed as required.
  16. Facilitate consumer-centered program development, including identification of possible adaptive equipment.
  17. Work with the Program Director in implementation of PBS supports.
  18. Support Director of Day Habilitation in development, modification and implementation of internal quality controls for the Day Habilitation program.
  19. Excellent organizational, communication, and writing skills. Knowledgeable in Word, Excel, internet access and/or similar software programs
  20. Review consumer progress to determine appropriateness of services.
  21. Provide supports to individuals with physical challenges, such as: lifting, transferring and ADL’s.
  22. Perform other related work duties as assigned by the supervisor.

EDUCATION and /or EXPERIENCE

High School Diploma or GED required. Bachelor’s degree in human services or related field preferred. A minimum of three years’ experience in human services. Must have supervisory experience managing a human services program, preferably one serving adults with developmental disabilities. 

CERTIFICATES, LICENSES, REGISTRATIONS 

Require valid driver’s license and satisfactory driving record. Pass Criminal Background Check (CORI).
