Grow is committed to providing comprehensive support for school-aged individuals as they transition into adulthood through our specialized Transition Services. This department integrates a variety of services designed to ensure a smooth, successful shift from school to adult life. Drawing on our extensive experience in serving individuals with developmental disabilities, we provide a comprehensive approach that ensures a smooth transition into adulthood. Our team of experts utilizes proven strategies and best practices to assess each student’s readiness, identify areas of need, and equip schools, parents, and students with the knowledge and tools necessary for success.
- Transition Assessment: The Transition Assessment is designed to support school-aged students transitioning from school to adult services. This comprehensive assessment evaluates key areas such as vocational skills, behavior plans, leisure skills, ADL/life skills, and safety to ensure students are prepared for greater independence in adulthood. Using interviews, observations, and assessment tools, staff gather information to create an individualized report with recommendations for skill development, behavior plan adjustments, and strategies for maximizing independence. These recommendations ensure a smooth transition and readiness for adult services.
- On-Site Transition Programming: This program offers transition-age students the chance to integrate into Grow’s established adult service programs, gaining hands-on experience to support a smooth transition into adulthood. Students will develop essential skills in vocational training, life skills, and community safety. This integration fosters independence and confidence, ensuring students are fully prepared to navigate and succeed in adult services.
- Transition Training: These trainings equip schools and parents with the essential knowledge and tools to successfully support individuals transitioning into adult services. These trainings focus on the critical differences between school-aged and adult services from the perspective of an adult service agency. Key topics include IEP development for a successful transition, life skills goals for adulthood, goal setting for a smooth transition, an overview of adult services, and behavior management strategies tailored for adult environments. Our training helps ensure that educators and families are prepared to navigate the complexities of this transition and provide the best support for each individual’s future success.
