Bridging the Gap: Navigating the Transition from School to Adult Services

Join us for a practical and informative training designed to help families, educators, and professionals navigate the transition from special education to adult services. Led by experienced adult service provider, this session will offer a behind-the-scenes look at what happens after age 22—and how to prepare.
We’ll cover key topics including:
- An overview of adult day program options including CBDS, Employment Services and Day Habilitation
- The critical skills to build before the transition
- Tips for creating a smooth, person-centered transition plan
